Insurance

All major insurance companies are accepted, including Medicare. Please call the office directly to verify that we take your insurance. If we do not take your insurance we will review our cash payment option.

Insurance Verification
We will call to verify your benefits but it is never a guarantee of coverage. The benefits that your insurance has given us will be reviewed with you on your initial visit.

Insurance Billing
As a courtesy, we will bill your insurance company if you have one. Ultimately however, your bill is your responsibility regardless of insurance coverage. We encourage you to learn about your individual coverage and how it applies to physical therapy.

Co-Payment / Co-Insurance
Your co-payment is due at the time of service each and every visit. For your convenience we accept cash, checks, Visa and MasterCard.

Deductible
If you have an insurance deductible, we are legally required to bill you for the full amount of each treatment until you have met your deductible. If you have paid charges to other providers that were not yet posted when your insurance was verified, you will be entitled to a refund when your insurance company’s explanation of benefits (EOB) indicates a refund is due.

Lymphedema Patients
Effective August 03, 2009
Tina L Baum Physical Therapy is no longer providing compression bandages and compression garments to our lymphedema patients. We have opted to refer patients to a medical supply company to make these purchases.We have had to make this policy change because insurance companies are either not covering our costs or it is a non-covered service. We apologize for any inconvenience that this may cause.
