What can I expect on my first visit to Tina L Baum
Physical Therapy?
Please arrive 30 minute prior to your evaluation
in order to complete the appropriate paper work
(or complete it at your convenience by
downloading forms
here and arrive 15 minutes prior to your
appointment).
Our office staff will review your physical
therapy benefits. You will then be evaluated by
one of our licensed physical therapists. Your
evaluation will be a one-on-one session in which
the clinician will perform a thorough exam,
noting any areas of difficulty or limitation
relating to your problem or complaint. Based on
this evaluation, we will review our recommended
plan of care based on our findings and the
diagnosis supplied by your physician. At this
time, we will answer any questions that you may
have. You will be issued a home exercise program
that is individually prescribed by your
clinician for you.
What should I bring on my first visit?
You should wear or bring comfortable workout
clothes (including shoes and socks), your
prescription from your physician, your insurance
card and your co-pay/deductible if applicable.
How long will my visit take?
The first visit usually takes 1 to 1.5
hours. Follow-ups generally take 45 minutes to 1
hour. It is common for patients to come 2-3x/wk
for 3-6 weeks. Frequency and duration vary
greatly with the diagnosis, chronicity of the
problem and compliance with recommended home
program. Your physical therapist will review the
estimated frequency and duration of your visits.
Who is involved in my rehabilitation?
Your rehabilitation will be overseen by your
physician and supervised by your physical
therapist, with assistance from ancillary and
administrative staff. After a comprehensive
evaluation, goal assessment, and treatment plan,
you will be scheduled for a return visit(s)
based on your physician and clinician’s
recommendation. Tina L Baum Physical Therapy’s
primary objective is to help you achieve your
rehabilitation and treatment goals and prevent
further injury.
Will my physician be involved in my rehab?
The physician/therapist relationship is an
important one in order to effectively serve our
patients. After your evaluation and assessment,
a detailed copy of your evaluation will be sent
to your physician. Likewise, progress notes will
be sent to update your doctor on advancement
toward your stated goals. Tina L Baum Physical
Therapy will work diligently to educate you and
return you to your optimal physical conditioning
and performance level as soon as possible.
How can I provide feedback?
Please communicate your needs or questions to
your physical therapist, support staff or our
front office at any point. Tina L Baum Physical
Therapy takes all feedback from patients
seriously and appreciates your input.
